FAQs

🔸 General Rehab Services 🔸

What does Form and Focus Rehab do?

Form and Focus Rehab provides in-home physical and/or occupational therapy in Saint Clair Shores, Grosse Pointe, and surrounding communities to help adults regain strength, balance, and confidence in their homes and communities.

Do you come to my home for therapy?

Yes! We specialize in in-home physical and/or occupational therapy in Saint Clair Shores and Grosse Pointe, bringing personalized rehab directly to your home—no clinic visits needed. This allows us to tailor treatment to your real-life environment like, stairs, furniture, and daily routines—so progress carries over into your everyday life.

How soon can therapy start after a fall?

We aim to start therapy as quickly as possible—often within a few days of your inquiry. Early intervention after a fall can help prevent further injury and support faster recovery.

What makes in-home therapy different from outpatient therapy?

In-home therapy is more personalized, convenient, and functional. Instead of working in a clinic, we treat you in your own environment, focusing on real-life movements like walking safely, navigating stairs, and maintaining independence at home. There’s no need to travel.

🔸 Common Conditions Treated 🔸

What conditions do you treat with in-home physical/ occupational therapy?

We treat a wide range of conditions including Parkinson’s disease, stroke recovery, balance disorders, fall risk, orthopedic injuries, and general mobility limitations. Our in-home physical therapy services are tailored to your specific goals and environment.

Do you work with people who have Parkinson’s disease?

Yes! We specialize in working with individuals with Parkinson's disease, using movement-based therapy and evidence-based approaches, like LSVT Big and PWR! principles. Our goals is to improve mobility, balance, and confidence in everyday activities.

Who is a good fit for in-home physical/occupational therapy

In-home physical therapy is ideal for adults who have difficulty traveling, are recovering from injury or surgery, or want more personalized, one-on-one care. It’s especially helpful for individuals working on balance, fall prevention, or neurological conditions, like Parkinson’s disease, to maneuver around the home.

🔸 Getting Started 🔸

Do I need a doctor’s referral for therapy?

In Michigan, you can often begin physical therapy without a referral (direct access). However, some insurance plans may still require one—we are happy to help guide you through the process.

How do I schedule my first visit?

Just call, email, or fill out our contact form. We’ll discuss your goals and set up an initial in-home evaluation at a time that works best for you.

🔸 What to Expect 🔸

How long is each therapy session?

Most sessions last about 45 to 60 minutes and are customized to your goals and progress.

Will I get exercises to do on my own?

Yes! We provide simple, tailored, personalized home exercise programs to help you continue making progress between visits and build long-term strength and confidence.

🔸 Insurance & Payment 🔸

Do you accept insurance?

We are out-of-network with most insurance plans, but we provide superbills for reimbursement. Many clients also use HSA/FSA funds for payment.

How much does each session cost?

Our rates are based on the length and type of visit. We’re happy to discuss pricing transparently during your free phone consultation.

Katherine Andary

Doctor of Physical Therapy (DPT)

Based in Saint Clair Shores, MI, I provide mobile physical therapy services throughout the surrounding communities. I received my Bachelors of Health Science and Doctorate of Physical Therapy at Wayne State University in 2014. I found my passion right away working with people who have balance problems within the geriatric population. I like challenging them to feel confident in their abilities. Currently, I specialize in treating people with Parkinson's Disease (PD). I am certified in LSVT Big, PWR , and Fall Prevention. Parkinson's Disease is becoming more prevalent and the age of onset is occurring earlier. Every person with PD is different. I find joy in helping individuals go through the process of their initial diagnosis and guiding them through their journey. I also love treating those with any neurological diagnosis and orthopedic conditions.

In my spare time, I love to spend time with my husband, two boys, and Newfoundland dog. I continue to practice ballet and train for triathlons and adventure races.
