What does Form and Focus Rehab do?

Form and Focus Rehab provides in-home physical and occupational therapy to help adults regain strength, balance, and confidence in their homes and communities.

Do you come to my home for therapy?

Yes! We specialize in mobile therapy, meaning we bring personalized rehab directly to your home—no clinic visits needed.

How soon can therapy start after a fall?

We aim to start therapy as quickly as possible—often within a few days of your inquiry. Early intervention after a fall can help prevent further injury and support faster recovery.

What makes in-home therapy different from outpatient therapy?

In-home therapy is tailored to your actual living environment, which makes it more personalized and relevant. There’s no need to travel, and exercises are based on your real-life challenges like stairs, furniture, or layout.

Do you work with people who have Parkinson’s disease?

Yes. We provide movement-based therapy and balance training for clients with Parkinson’s. We also offer assessments to determine if higher or lower intensity exercise is appropriate.

Do I need a doctor’s referral for therapy?

In Michigan, you can start physical therapy without a referral. However, some insurance plans may still require one—we can help check.

How do I schedule my first visit?

Just call, email, or fill out our contact form. We’ll discuss your goals and set up an initial assessment at your home.

How long is each therapy session?

Most sessions last about 45 to 60 minutes and are customized to your goals and progress.

Will I get exercises to do on my own?

Yes! We provide simple, tailored home exercises between visits to keep you moving forward.

Do you accept insurance?

We are out-of-network with most insurance plans, but we provide superbills for reimbursement. Many clients also use HSA/FSA funds.

How much does each session cost?

Our rates are based on the length and type of visit. We’re happy to discuss pricing transparently during your free phone consultation.

Doctor of Physical Therapy (DPT)

I received my Bachelors of Health Science and Doctorate of Physical Therapy at Wayne State University in 2014. I found my passion right away working with people who have balance problems within the geriatric population. I like challenging them to feel confident in their abilities. Currently, I specialize in treating people with Parkinson's Disease (PD). I am certified in LSVT Big, PWR , and Fall Prevention. Parkinson's Disease is becoming more prevalent and the age of onset is occurring earlier. Every person with PD is different. I find joy in helping individuals go through the process of their initial diagnosis and guiding them through their journey. I also love treating those with any neurological diagnosis and orthopedic conditions.

In my spare time, I love to spend time with my husband, two boys, and Newfoundland dog. I continue to practice ballet and train for triathlons and adventure races.
